Registered Nurse
Care Coordination/Case Management

Due to our growth we are adding an RN Care Coordinator/Case Manager to our team.   This is an ideal position for the professional Registered Nurse who would like the opportunity to work in a clinical based case management practice, managing chronically ill patients in the community.  We offer autonomy and flexibility in the work environment.  We are looking for a candidate who is excited about being an integral part of a growing company in the emerging field of RN care coordination/case management.

We are looking to fill positions for both part-time (working up to 20 hours per week) and full-time employment. Benefits for full time positions may be considered.  The position is based in the Walnut Creek/Alamo and surrounding areas.

This critical position will be responsible for all aspects of clinical care coordination and case management for a client caseload.  We service patients with a wide variety of acute and chronic healthcare needs across the lifespan.

Major responsibilities include:

  • Initial consultation and intake for new clients
  • Patient physical assessment
  • Development of a holistic and comprehensive plan of care
  • Strong communication skills to act as liaison between the medical team, patient and loved ones
  • Preparation of weekly communications to medical team, patient and family
  • Patient, family and caregiver education
  • Medication management/in-home assessments/patient necessitated nursing tasks
  • Providing appropriate referrals based on patient needs
  • Providing support and coaching for office visits, including all appropriate documentation
  • Strong computer skills for electronic medical record system, Microsoft Word and Outlook

Job Requirements:  Applicants for this position must have the following skills and experience:

  • Current Registered Nurse licensed to practice in the state of California
  • Graduate of an approved and accredited nursing program
  • Preferred 5 years nursing experience, including acute care experience
  • Strong patient assessment skills
  • Current BLS certification
  • Outstanding written and verbal communication skills
  • Ability to be a strong but tactful advocate for patients
  • Ability to work independently
  • Knowledge of PDA style cell phone use such as iPhone preferred
  • Computer literate with strong MS Office skills including MSWord, Excel and Outlook
  • Valid CA driver’s license, current automobile insurance and access to a reliable vehicle
  • Proof of personal RN liability insurance
  • Experience as charge nurse and/or supervisory experience preferred
  • RN Case Management and Community RN experience a plus
  • Certification in Case Management (i.e. through CMSA) a plus
